Presenter:

Brittany Sheldon, M.S., CCC-SLP, is a Speech-Language Pathologist with almost two decades of experience across all school settings. Currently, Brittany is the Regional Director of Clinical Services for multiple regions of The Stepping Stones Group. As a part of that role she manages a multi-disciplinary group of Clinical Managers, a team of school-based SLPs, SLPAs, OTs, and PTs, as well as a handful of school district partnerships. As a manager, she aims to establish a sense of community, unity and intrinsic value amongst the area managers. As an SLP, her approach to care includes comprehensive assessment, interprofessional practice, and therapeutic integrity using evidenced-based practice.

As a result of this seminar, participants will be able to:

  1. List the four K-12 transition periods of a student’s educational career as outlined and defined by IDEA 2004.
  2. Identify at least 1 key consideration for each of the four transitional periods to support effective planning.
  3. Describe at least 2 common barriers to creating and implementing transition plans and at least one strategy to overcome each barrier.
  4. Explain the importance of the multidisciplinary team working collaboratively to support positive student outcomes during transition to post-secondary.
