Presenter:
Brittany Sheldon, M.S., CCC-SLP, is a Speech-Language Pathologist with almost two decades of experience across all school settings. Currently, Brittany is the Regional Director of Clinical Services for multiple regions of The Stepping Stones Group. As a part of that role she manages a multi-disciplinary group of Clinical Managers, a team of school-based SLPs, SLPAs, OTs, and PTs, as well as a handful of school district partnerships. As a manager, she aims to establish a sense of community, unity and intrinsic value amongst the area managers. As an SLP, her approach to care includes comprehensive assessment, interprofessional practice, and therapeutic integrity using evidenced-based practice.

- List the four K-12 transition periods of a student’s educational career as outlined and defined by IDEA 2004.
- Identify at least 1 key consideration for each of the four transitional periods to support effective planning.
- Describe at least 2 common barriers to creating and implementing transition plans and at least one strategy to overcome each barrier.
- Explain the importance of the multidisciplinary team working collaboratively to support positive student outcomes during transition to post-secondary.
